Only CPT codes, descriptions and other data are copyright 2012 American Medical Association (or other date of publication of CPT). All rights reserved. Applicable FARS/DFARS clauses apply.
You, your employees and agents are authorized to use CPT only as provided in the following authorized materials (web pages, PDF documents, Excel documents, Word documents, text files, Power Point presentations and/or Flash media) contained internally within your organization within the United States for your sole use, employees and agents. Use is restricted to use in Medicare, Medicaid, or other programs administered by the Centers for Medicare & Medicaid Services (CMS). You agree to take all steps necessary to ensure that your employees and agents comply with the terms of this Agreement.
Any use not authorized herein is prohibited, including, by way of illustration and not limitation, making copies of CPT for resale and/or licensing, transferring copies of CPT to any third party not bound by this Agreement, making modified or derivative works of CPT or commercial use of CPT. License to use CPT for any use not authorized herein must be obtained through the AMA, CPT Intellectual Property Services, 515 N. State Street, Chicago, IL 60610. Applications are available on the AMA website,http://www.ama-assn.org/go/cpt.
Applicable FARS/DFARS restrictions are for government use.
